Membership Authorization:

Yes, I want to join my fellow Prosecutors and reaffirm and/or apply for membership in the Connecticut Association of Prosecutors.

I hereby reaffirm and/or apply for status as a member and wish to continue as a member with all the rights of members under the By-Laws of the Connecticut Association of Prosecutors and under law.

I authorize the Connecticut Association of Prosecutors to act as the duly certified exclusive representative in collective bargaining over wages, benefits and other terms and conditions of employment with my employer, the Division of Criminal Justice, State of Connecticut.

Dues Deduction and Checkoff Authorization:

I recognize the need for a strong union and join with all the other members to support our union in seeking the best possible pay and benefits and working conditions for State of Connecticut Prosecutors. I hereby request and authorize my employer, the Division of Criminal Justice, State of Connecticut, to continue to deduct from my earnings and pay over to the Connecticut Association of Prosecutors the regular bi-weekly dues uniformly applicable to members of the Connecticut Association of Prosecutors. This authorization shall remain in effect unless I revoke it by sending a written notice via U.S. Mail to the Connecticut Association of Prosecutors not less than thirty(30) days and not more than forty-five(45) days before the one year anniversary date of my submission of this authorization to the Connecticut Association of Prosecutors or the date of the termination of the current contract between my employer and the Connecticut Association of Prosecutors, whichever occurs sooner. This authorization shall be automatically renewed from year to year unless I revoke it in writing as outlined herein.
